Terms and conditions for Small Group Sessions - 12 Sessions/Month

Plan is in force for 3 months from the Plan Start Date, with automatic renewal for subsequent months, unless terminated by either Client or Pilates by Wendy, LLC d/b/a Sentirsi Studio (herein after referred to as “the Studio.”) Client may terminate Plan with notification at least 15 days in advance of the next billing date; Termination fee applies - see (h) below. Plan may be terminated at any time by the Studio.

Client agrees to pay the Studio monthly recurring fees as follows:

"Small Group Sessions - 12 Sessions/Month" Plan for $385/month via Client authorized automatic credit card charges. Payment in monthly increments by any means other than Client's credit card is not permitted.

Please read and sign below.
(a) I agree to purchase the "Small Group - 12 Sessions/Month" Plan for $385 as an automatic charge to my credit card each month.
(b) I understand that this Plan is valid for virtual sessions.
(c) I understand I will be charged for any Session that is Late Cancelled (cancelled within 24 hours of start of the Session.)
(d) I hereby certify that I am the holder of the credit card provided to the Studio.
(e) I understand that I will be notified if my credit card payment fails to authorize for any reason, and that a $10 late fee will apply if I do not provide a valid credit card information within 10 calendar days of the original rejection date.
(f) I understand that my Plan will be deactivated if my account becomes more than 10 calendar days late.
(g) I understand that I can take up to 12 Small Group Sessions (aka Virtual Classes) per month. Up to 3 unused sessions may "roll over" to next billing month.
(h) I understand that I may terminate the "Small Group Sessions - 12 Sessions/Month" Plan by submitting a written Plan termination request to the Studio at least 15 days prior to my next billing date. I understand that if I terminate the Plan prior to the completion of the 3-month contract period, a $193 termination fee applies and will be automatically charged to my credit card.

(i) I understand that my Plan may be frozen for up to 14 days if I am ill from, diagnosed with or have been exposed to Covid-19 or up until at least 72 hours have passed since the resolution of fever without the use of fever-reducing medications, and improvement in respiratory symptoms such as shortness of breath or cough, and at least 7 days have passed since symptoms began. There is no fee for this freeze.

(j) I understand that all sales are final and that no refunds will be provided for this Plan. There are no refunds for unused sessions.

(k) I understand that my Plan is NOT transferable to another person.

By signing below, I authorize autopay and agree to the terms and conditions of the "Small Group Sessions - 12 Sessions/Month" Plan
